Care Needs Assessment

Do I need to get a ‘Care Needs Assessment’ for my loved one?

Yes, this is your first step. If you or a loved one is finding it difficult to manage daily tasks like cooking, dressing, or getting around, the first step towards getting the right support is a care needs assessment.

How do I start a Care Needs Assessment?

Simply call our customer service manager for an informal chat about the type of care you feel might be required. We’ll forward you the forms you need and, when you’ve completed them, we’ll arrange for one of our expert care professionals to contact you to arrange a personalised assessment.

What to expect during the assessment

Our care professional will visit, either at home or in hospital, to talk about the current situation.
They’ll look at things like:

  • Managing tasks such as medication, washing, eating and getting dressed
  • Mobility and access to the local community
  • Physical and mental health
  • Goals and preferences

They may also speak to the GP or other healthcare professionals to get a full picture of needs.

How to prepare for a Care Needs Assessment

Think about where support is needed. Be honest and detailed, for example:

  • “I/they need help getting out of bed in the morning.”
  • “I/they struggle to prepare meals or remember to take medication.”
  • “I/they feel isolated and don’t get out much anymore.”

The more you share, the better we can understand your situation.

After the assessment

You should receive the results within a week. If they’re eligible for support, the next step will be a financial assessment (means test) to see how much you may need to contribute. If you’re unhappy with the outcome, there are organisations that can offer independent advice and support.
